Fill out the form with your personal information and teacher information
Upload your hand-drawn thank you note (no computer-generated graphics are allowed) – limit 10 entries per person per email address

Every Tuesday through August 28th, five prize packs will be awarded which will include a $500 gift card for the teacher AND a $500 gift card for the nominator. All Thank You cards submitted in the week prior to each drawing are eligible for this prize. Since you can enter up to 10 times, you might want to enter once a week!
